AGE

Your child must be two (2) years old on or before December 31st of the current year in order to enter the 2 year old class.

Your child must be three (3) on or before December 31st of the current year in order to enter the three year old class.

The child entering the four year old class must be four (4) on or before December 31st of the current year.

The child entering Kindergarten must be five (5) on or before December 31st of the current year. This requirement is different than the Florida public school system and could result in the child repeating Kindergarten if transferred to a public setting the next year.

Children entering 1st grade must be 6 years old on or before December 31st of the current year or show proof of having completed a recognized Kindergarten program. Individualized placement testing will be utilized if placement is in question.

All children, except those entering the two (2) year old class MUST be fully toilet trained.

HEALTH

A health form (Form HRS-H3040-March 91) completed by the child's physician must be filed with the Director. We cannot admit any child without these records.

All immunization shots recommended by the child's physician and required by the Health Department should be given and recorded on the proper forms (Blue HRS 680) and also turned in before the child can be admitted.

Immunizations must be kept up-to-date or your child must be withdrawn until the shots are updated.

BIRTH CERTIFICATE

It is a requirement of Sunrise Christian Academy that a copy of the child's certified birth certificate be submitted at registration. In lieu of a copy of the birth certificate, the parents may submit a notarized Affidavit of Birthdate, which is available through the Academy office.
